Patient Registration

Please check your postcode on the Practice Area Map page to see if your address falls within the practice boundary. You will be asked to complete a health questionnaire and provide some form of identification. You will also need to provide proof of your address. Please speak to a member of the reception team if there are issues with your proof of address or ID.

Everyone over 16 years of age will be given an appointment with a member of the nursing team. She will ask about your past medical history and current medication to assist in assessing your health needs. A brief health check will be performed so please bring a specimen of urine with you. In addition you will be asked to provide details of childhood immunisations of children under six years of age (this will ensure that your child is included in the computerised recall system operated within NHS Walsall).

Please download and complete the New Patient registration form. To register please bring the completed form in to the surgery.

Patient Registration Form

OUT OF AREA REGISTERED PATIENTS WITHOUT HOME VISITS

From January 2015 Field Road Surgery and other Practices, are able to register a new type of patient called OUT OF AREA PATIENTS WITHOUT HOME VISITS. Patients can join the practice list that live outside the practice boundary, but home visits will be provided by another Practice nearer to their home. This type of registration might suit someone who works in Bloxwich but lives further afield for example.Check you postcode via the Practice Area Map page to see if you will be eligible for this type of registration.

Patients will be asked to fill in a new patient registration form and await the considered response from Dr Ahmed and Dr Saleem. The decision to accept patients for the new kind of registration is at the Doctors discretion based on clinical grounds. Because of the greater distance to your home, the GP is under no obligation to offer you a home visit. If you are not well enough to go to the practice yourself then other arrangements will have to be made. NHS England (the body responsible for buying GP services) has to ensure that you are able to access a service either near your home or at home. If you register with a practice away from home for these purposes then you will be given information about what you should do in those circumstances.

ANY PATIENT ACCEPTED ONTO THE PRACTICE LIST LIVING OUTSIDE THE PRACTICE BOUNDARY WILL HAVE THEIR HOME VISIT REQUIREMENTS PROVIDED BY A GP IN THEIR AREA AND NOT BY FIELD ROAD SURGERY.

PATIENTS WHO DO NOT HAVE HOME VISIT REQUIREMENTS PROVIDED BY A GP IN THEIR AREA, UNFORTUNATELY, WILL NOT BE ABLE TO JOIN FIELD ROAD SURGERY.

Our practice is inspected by the Care Quality Commission (CQC) to ensure we are meeting essential standards of quality and safety.

This widget provides a summary of the results of the latest checks carried out by the CQC.
